A butcher was chased around his yard by a sword-wielding thug who threatened to kill him.
Paul O'Neill also sprayed petrol as the butcher Ryan Moffat ran for his life, Hamilton Sheriff Court heard on Friday.
But it was O'Neill who landed in hospital after he suffered serious head injuries when he was struck by a car in the yard.
O'Neill, a prisoner, admitted assault, threatening or abusive behaviour and illegal possession of a Samurai sword in Station Road, Shotts, on July 9.
The 40-year-old also admitted punching a female doctor at Wishaw General and committing the offences while on bail.
Aga Mathieson, prosecuting, said the trouble started around 10am when Mr Moffat found O'Neill in the backyard of the family shop and asked if he could help him.
She told the court: "The accused became annoyed and threw a lighter that hit a wall.
"He swore at Mr Moffat who believed he was going to be attacked and punched the accused, causing him to fall.
"O'Neill got up and left but returned 10 minutes later and shouted, 'do you know who I am?'
"He took a Samurai-style blade from a plastic bag he was carrying and brandished it.
"When the victim saw this, he went behind a car and the accused began to chase him, shouting 'I'll fu**ing kill you'.
"O'Neill was also carrying a small bottle containing petrol that he sprayed at the victim.
"A passer-by entered the yard to see what was happening. O'Neill tried to leave but was struck by a car.
"The police and ambulance were called and he was taken to hospital with very serious wounds to his head."
The doctor examined O'Neill at the hospital, but when she told him to keep still he replied, 'you're hurting me so I'll fu**ing hurt you'.
He punched the doctor on the face before police officers restrained him.
O'Neill has been in custody since his arrest and Sheriff Liam Murphy told him that, given the nature of the offences and his previous convictions, a prison sentence was "inevitable".
The case was adjourned until next month for background reports.
