A firefighter rushed home and performed CPR on his dying wife after hearing his own address on the scanner while on duty.
Walter Stewart's wife and his three young children succumbed to the injuries they suffered in a fire at their home in Chicago, Illinois, US, on March 7.
Emergency crews rushed to the burning home and took the family to the hospital, while an investigation into the circumstances under which the blaze broke out is ongoing.
His wife, Summer Day-Stewart, 36, died on Friday, while their 7-year-old son, Ezra Stewart, died on Wednesday.
The two remaining children, a two-year-old boy and a 9-year-old girl, who had been hospitalised since the fire, passed away on Friday night.
It was reported that the fire started in the home's kitchen, although the investigation was still ongoing and it is not yet known whether the incident was intentional or not.
The damage to the home was significant, and a deceased pet was found inside the house.
Ignite the Spirit, a Chicago Fire Department charity, has established a fund to support the family.
The "Stewart Family Fund" page reads: "The Stewart Family is facing an unspeakable tragedy.
"Walter Stewart, a Chicago Firefighter, was on duty at the firehouse when he overheard his home address on the fire radio, indicating that an active fire was raging.
"When he arrived on the scene he found his house in flames and his wife and children in grave condition.
"Tragically, Walter lost his Wife and three children. Please keep the Stewart Family in your thoughts and prayers."
A fire department source told the Chicago Sun-Times that Mr Stewart agreed to donate organs from all four family members so that others who need transplants could live.
The fire department first notified fellow members about whether anyone was in need of a donation, the source said.
At the time of the incident, a neighbour, Mildred Bran said he heard a loud bang and looked out to see the home fully engulfed in flames.
He told ABC 7: "I was shocked. There was so many firefighters leaving, so many police. It's crazy. It hits so close to home, right across the street."
Another neighbour, Joel Rivera described the scene saying: "It was just smoke, really. The smoke that I saw visibly was smoke coming from the top of the home.
"I did see what appeared to be an adult being resuscitated. It did not look like that person was in good condition."
