Megan Jayne Somerville faced the Adelaide Magistrates Court via video link on Thursday.
It is alleged the 35-year-old stopped her car on a motorway in August, got out with her two sons, aged three and eight, and started assaulting them with a knife.
She was disarmed by a member of the public and detained until police arrived.
The two children were taken to the Women's and Children's Hospital in a critical condition.
They both required surgery after the incident.
Police later said the children were in a stable condition and expected to recover.
At the time, police praised the efforts of the passer-by, saying the situation "could have been much worse".
The court heard there would be a mental incompetence defence raised in the Supreme Court.
Prosecutor Peter Cannell told the court he would be asking for a fast-tracked committal of the accused to the Supreme Court.
"There is a case to answer on the information already provided," Mr Cannell said.
"And there is further outstanding material."
Ms Somerville will be arraigned in the Supreme Court in February.
She is being held in a secure mental health facility.
